Book Appointments By Phone or Fax

Appointments can be booked by your clinic over the phone, or faxed to Vita at 587-393-8773. Your clinic will receive a faxed confirmation from us for specialist consultation referrals and you will also receive a faxed confirmation for the dates and times of all appointments scheduled for your patients.

Online Booking (Physician Portal Only)

We are excited to announce we are now offering online booking to select clinics for the following testing modalities:

• Echocardiograms

• Electrocardiogram (ECG)

• Holter Monitors (24 HR ONLY please send all 48 hour HM requests by fax)

We will be rolling this feature out slowly, however if your clinic would be interested in having access to our online booking portal please contact us directly at info@vitadiagnostics.ca.

All patients booked online must provide an email address where we can send confirmation of appointment.

Referral Forms

In our efforts to making referring to our clinic as seamless as possible, our requisition forms can be found in most common EMR's such as Telus Health's Wolf, Med Access, PS Suites, Accuro and Healthquest. Alternatively, you may request paper requisition pads by emailing hr.vitadiagnostics@gmail.com or you can simply click the download link below and print as you need.

Cardiology Consultations

Please indicate on our requisition form or in a written letter if you would like to book your patient for a consultation with our next available cardiologist. Please attach prior testing results with your referral if performed elsewhere.
